Financial policy
Nine terms covering payment, insurance processing, fees and overdue accounts.
- Our practice is committed to providing the best treatment for our patients and charges what is usual and customary in our area. The patient or guardian remains responsible for payment regardless of an insurance company’s determination of usual and customary rates.
- You will receive a receipt for payments made by cash or check.
- If you do not have a proper insurance ID card showing the doctor’s and patient’s names, or if coverage cannot be confirmed at the time of your visit, payment is expected at the visit. The office will issue a refund after the insurer reimburses the practice.
- Copays, balances and deductibles are due before the patient is seen.
- An appointment cancelled without 24 hours’ notice may be charged an additional $25.
- If payment is not received by the due date, a late fee of up to $50 for each visit may be charged beginning 14 days after the patient or parent is notified in person, by mail or by phone.
- If incorrect insurance information prevents the practice from billing within the insurer’s time limit, the patient or guardian is responsible for payment when the insurer does not compensate the practice.
- A returned check may be charged $35 in addition to the remaining balance.
- A delinquent account may be responsible for additional billing costs. If assigned to a collection agency or attorney, additional costs of 35% to 50% may apply depending on the situation.
Insurance & referral policy
Three terms covering referrals, outside services and vision benefits.
- When a referral is required, the practice may need up to three business days to prepare it unless the physician determines there is an emergency. The practice is not responsible for an insurer’s delay or denial, does not issue backdated referrals, and asks families to collect an approved referral before the specialist visit.
- Families are responsible for understanding coverage and benefits. Before going elsewhere for blood work, imaging, a specialist or another service, confirm with the insurer and the outside facility that the visit will be covered.
- For eyeglasses or contact-lens checks and rechecks, confirm whether your insurance includes a vision benefit and follow that plan’s requirements.
Forms, records & appointments
Eight additional terms covering paperwork, records, prescriptions, appointments and practice communication.
- Forms, reports and other paperwork may require up to three business days and, in some cases, a fee paid in advance based on the preparation involved.
- Medical record or document photocopies may be charged at $1 per page where permitted by the New Jersey Department of Health.
- The first medical-record summary is provided without charge after a written request and may take seven days. It must be picked up rather than mailed or faxed. A repeat summary may cost $25, and an insurance company requesting records may be charged $50.
- The captured office policy states that prescriptions are not called in over the phone. Contact your office for the current medication-request process.
- Notify the practice promptly of changes to insurance, phone numbers, home or work addresses, or other important information.
- If the practice believes there are unmanageable differences affecting the physician–family relationship, it may ask the family to choose another physician. In that situation, emergency-only care may be provided for 30 days while the family establishes care elsewhere.
- Patients are seen by appointment. Please call rather than walking in. If you are running late, call the office; arrivals more than 20 minutes late may need to be rescheduled depending on the day’s schedule.
- Current policy changes may be posted in the offices from time to time.
